Background: Pressure ulcers are wounds caused by prolonged pressure in patients with limited mobility, requiring effective wound dressings to accelerate tissue regeneration. A non-adhesive hydrogel foam dressing based on sodium alginate combined with binahong (Anredera cordifolia) leaf extract has the potential to promote wound healing through its biocompatibility, moisture-retention capacity, and anti-inflammatory as well as regenerative properties. Objective: This experimental study aimed to evaluate the wound-healing activity of a non-adhesive sodium alginate-based hydrogel foam dressing containing binahong (Anredera cordifolia) leaf extract in a Wistar rat pressure ulcer model. Methods: The study included formulation of the hydrogel foam dressing, phytochemical screening of secondary metabolites, evaluation of its physical characteristics (organoleptic properties, pH, folding endurance, gel fraction, and swelling ratio), and assessment of wound-healing efficacy based on wound area reduction and histopathological examination. Results and Discussion: Binahong hydrogels containing 5–15% extract showed a pH of 6.05–6.12 and a thickness of 0.82–0.91 mm. Increasing the extract concentration enhanced swelling (4.82–6.13), gel fraction (78.35–84.92%), and folding endurance (126–151 folds). The 15% formulation achieved 98.75% wound healing with a wound diameter of 0.25 ± 0.22 mm on day 21. One-way ANOVA revealed significant differences among groups (p < 0.001). Histopathological evaluation of K6 showed a collagen score of 3 and an inflammation score of 2, indicating improved tissue repair. Conclusion: The non-adhesive sodium alginate-based hydrogel foam dressing containing binahong (Anredera cordifolia) leaf extract shows promise as an effective and cost-efficient alternative wound dressing for the treatment of pressure ulcers.
